HOW TO ACCESS OUR SERVICES

To receive government subsidised services, you must be registered and assessed by My Aged Care. You can either:
• Call My Aged Care on 1800 200 422 – if you need help with this, please ask; or
• Go to myagedcare.gov.au; or
• Call us on 9841 8668 to make a referral on your behalf (My Aged Care will still contact you or your representative to discuss your needs)

After registering with My Aged Care, you will be assessed by the government assessment team as either eligible to receive CHSP (Commonwealth Home Support Program) entry level services or HCP (Home Care Package) services.

Select Albany Community Care as your preferred provider, and we will arrange a meeting with you to discuss your needs and preferences, and create your care plan.

If you are not eligible for government subsidies, or you don’t want them, you can still receive services from us as a private client.
All the same Quality Standards and how we deliver your services will remain the same, however you will be required to pay the whole cost of the service yourself.

FUNDING TYPES AVAILABLE
Private Clients
If you are not eligible for Government funded care, you can receive our services as a private client. Because private services are not subsidised by the Government, you will need to pay the full cost of your services, which are matched to your care needs and financial situation.
Private services can provide you with quality assistance in your home to ensure you can continue to enjoy your favourite activities either ongoing or whilst waiting for your Home Care Package to be assigned.

Commonwealth Home Support Programme (CHSP)
As you get older, everyday tasks can become more difficult. Funded by the Australian Government, the Commonwealth Home Support Programme provides a range of entry-level aged care services for older people to keep living independently at home and in their community.
The program has a focus on working with you, rather than doing for you, so you can continue to perform daily tasks independently for as long as possible.
It is about building on your strengths and abilities to help you remain living independently and safely at home.
Home Care Packages (HCP)
A Home Care Package is a coordinated package of care and services to help you to live independently in your own home for as long as possible. Some people receive services through the Commonwealth Home Support Programme and are then assessed for a Home Care Package. Others will start with a Home Care Package.
There are four levels of Home Care Packages to help meet the different levels of care needs. During your aged care assessment, your assessor will discuss your current care needs and determine the best level to meet them.
